Role: Registered Nurse On Call – providing after-hours hospice care in patients’ homes and facilities.
Responsibilities:
- Triage calls, assess patient needs, and make urgent visits as required.
- Manage symptoms and pain to ensure patient comfort.
- Implement and update individualized care plans in collaboration with the interdisciplinary team.
- Administer medications safely and accurately.
- Conduct patient assessments and document all care activities thoroughly.
- Educate families and caregivers on care procedures and symptom management.
- Support patients and families during crises, promoting dignity and comfort.
Qualifications & Skills:
- Valid Registered Nurse (RN) license.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and family education abilities.
- Ability to work independently with compassion and professionalism.
- Commitment to patient-centered, end-of-life care standards.
