Provide person-directed nursing care to skilled nursing residents, supporting rehabilitation, independence, and active participation in care.
Key Duties
- Assess residents’ nursing needs and respond to changes in condition.
- Implement and deliver nursing services aligned with individualized care plans.
- Document patient care accurately and maintain required records.
- Collaborate with the interdisciplinary care team to coordinate services and support goals.
- Provide occasional supervision and guidance to caregivers as needed.
Role Requirements
- Registered/licensed nurse capable of providing direct resident care in a skilled nursing setting.
- Strong assessment, clinical judgment, and patient-centered communication skills.
- Proficiency in nursing documentation and following care protocols.
- Ability to work independently while responding to urgent resident needs.
- Willingness to support on-call responsibilities and maintain weekend availability.
