Role Overview:
Seeking a dedicated Occupational Therapist to provide home health services, including assessment, evaluation, and treatment planning based on patient needs and medical conditions. The therapist will evaluate functional status, activities of daily living, and other occupational therapy requirements, collaborating with physicians and healthcare team members to develop and implement personalized care plans.
Key Responsibilities:
Deliver services within the scope of practice, adhering to state laws and evidence-based techniques
Assess patient functional status and coordinate with physicians for care planning
Monitor and document patient responses and condition changes
Conduct diagnostic and prognostic functional ability tests during patient assessments
Qualifications & Skills:
Current Occupational Therapy licensure and CPR certification
Valid driver’s license, vehicle insurance, or access to reliable transportation
Ability to work effectively in diverse home environments
Preferred: strong multitasking, independence, communication, and organizational skills
