Position Overview

An Occupational Therapist in home health provides personalized, one-on-one care to help patients live independently and safely at home. Responsibilities include conducting initial and ongoing assessments of mobility, strength, and function, and developing individualized plans of care with measurable goals and outcomes. The role involves delivering therapy services per physician orders, documenting progress, and modifying treatment plans as needed. Discharge planning and educating patients and caregivers are essential. The therapist collaborates with healthcare teams, maintains accurate documentation, and supervises relevant personnel such as OTA and HHA. Completing OASIS assessments and ensuring compliance with policies are also key duties.

Qualifications & Skills

  • Degree in Occupational Therapy from an accredited program
  • Unrestricted state license in practice location
  • Proficiency in assessments, documentation, and compliance
  • Valid CPR and Driver’s License

Preferred Experience

  • Home health therapy experience and OASIS documentation
