Home Health Occupational Therapist (OT)

Provide personalized occupational therapy to patients in their homes, helping them regain independence and improve daily functioning. Collaborate with a home health team including nurses, therapists, and aides to develop and implement care plans aligned with patient goals. Conduct assessments, establish therapy objectives, and document progress, focusing on enhancing fine motor skills, coordination, and home safety. Educate patients and families on exercises, adaptive equipment, and safety strategies, including fall risk reduction. Qualifications include a graduate from an accredited OT program, active CA OT license, and at least one year of experience, preferably in home health. Must possess CPR certification, knowledge of home health regulations, and demonstrate clinical judgment, compassion, and interpersonal skills.
