Occupational Therapist (OT) – Live Well Homecare

We are seeking a full-time Occupational Therapist (OT) to work in a Skilled Nursing Facility, with a $4,500 Sign-On Bonus.

Responsibilities include:

Evaluate residents and develop individualized OT treatment plans

Implement activities to promote residents’ independence in daily tasks

Assess, observe, and document therapy progress through detailed reports

Maintain resident confidentiality and foster team collaboration

Qualifications and Skills:

Graduate of an accredited Occupational Therapy Program

Current New York State OT License or actively pursuing licensure

Experience in skilled nursing or geriatrics preferred, but new graduates are encouraged to apply
