Role Overview: The Home Health Occupational Therapist assesses and develops personalized rehabilitation programs to improve patients’ strength, coordination, and independence within their homes. They collaborate with healthcare teams to create and adjust care plans, provide therapeutic treatments, and educate patients, families, and staff on rehabilitation techniques. The therapist also designs community reintegration activities, recommends adaptive equipment, and supervises therapy assistants and aides. Accurate documentation and communication of patient progress are essential responsibilities.
Qualifications & Skills: Candidates must hold current and unrestricted OT licensure, CPR certification, and possess a valid driver’s license with reliable transportation. Preferred experience includes a minimum of six months in occupational therapy and home health settings.
Key Duties:
