Role: Occupational Therapist specializing in home health care, delivering age-appropriate rehabilitative interventions to meet patients’ physical, emotional, developmental, psychosocial, and educational needs.
Responsibilities include:
- Assessing and managing patients’ rehabilitative needs, developing and revising care plans in collaboration with the care team.
- Providing direct patient care through therapeutic exercises, assessments, and treatments, ensuring quality and productivity standards.
- Coordinating services with team members and reporting patient progress and changes to physicians.
- Educating patients and caregivers to promote independence and support recovery.
- Participating in staff training, continuing education, and compliance with regulatory guidelines.
- Accurately documenting interventions and maintaining communication within the care team.
Qualifications:
- Graduate of an accredited Occupational Therapy program.
- Valid Occupational Therapist license and Basic Life Support certification.
- Minimum of one year full-time occupational therapy experience; home health experience preferred.
- Valid driver’s license and private vehicle insurance.
