Occupational Therapist – Live Well Homecare

Role: Occupational Therapist supporting older adults in various care settings, including assisted living, skilled nursing, memory support, and rehab.

Responsibilities:

Assess residents’ functional abilities and develop individualized treatment plans.
Provide evidence-based interventions to enhance independence, safety, and quality of life.
Collaborate with an interdisciplinary team to ensure comprehensive care.
Educate families and caregivers on treatment approaches and progress.
Document clinical outcomes and progress regularly.
Contribute to a compassionate, relationship-centered culture grounded in Christian values and purposeful living.

Qualifications & Skills:

Relevant licensure and experience in occupational therapy.

Strong assessment and treatment planning skills.

Excellent collaboration and communication abilities.
