Occupational Therapist – Live Well Homecare

Role Overview:
This occupational therapist assesses, plans, and implements rehabilitative programs to improve patients’ daily activities, mobility, cognition, strength, coordination, and range of motion. They provide care across the continuum, ensuring quality, cost-effectiveness, and regulatory compliance.

Key Responsibilities:

Implement and revise treatment plans with patient and family input

Educate patients and provide discharge instructions and follow-up

Collaborate with physicians to establish and evaluate treatment goals and discharge plans

Document evaluations, treatment sessions, and discharge information per departmental guidelines

Adhere to ethical standards, maintain confidentiality, and demonstrate effective communication

Participate in professional development, team activities, and mandatory training

Qualifications and Skills:

Bachelor’s and Master’s in Occupational Therapy
Active OT license and BLS certification
Effective communication, customer service focus, team orientation, and basic computer skills
