Seeking a Nurse Practitioner to deliver equitable, value-based primary care to Medicare adults in outpatient settings. Responsibilities include conducting office visits, wellness exams, care coordination, population health initiatives, patient education, and home visits as needed. The role involves collaborating with a multidisciplinary team and partnering with specialists and hospitals for comprehensive care.
Qualifications include a Master’s or DNP, national certification in family, adult, or gerontological primary care, active NP license, DEA license, and US work authorization. Preferred experience in primary care and familiarity with HCC, ICD-10 coding, and HRAs. Bilingual skills are a plus.
