Nurse Practitioner – Pulmonary – SNF / LTC Rehab center

We are seeking an experienced APRN in pulmonary care for part-time, in-person work in skilled nursing and long-term rehabilitation settings, requiring availability 2 days/week, including weekends, with occasional on-call.

Provide comprehensive pulmonary services, including assessments, management of respiratory conditions, and ventilator/trach care.

Manage admissions, discharges, and coordinate interdisciplinary care with physicians, respiratory therapists, and staff.

Prescribe medications, interpret diagnostics, and educate patients and families on respiratory therapies.

Ensure compliance with regulations, participate in quality initiatives, and maintain accurate documentation.

Qualifications include an active APRN license, Master’s degree, minimum 6 months in long-term care, 3+ years nursing experience, BLS/ACLS, and proficiency with EMR systems.

Compensation ranges from $60,000-$90,000 annually, with benefits like paid time off, malpractice insurance, ongoing education, and technology-enhanced documentation support.
