Nurse Practitioner – Pulmonary – SNF / Long-Term Care

We are seeking an experienced APRN in pulmonary care to provide high-quality, patient-centered services in skilled nursing and long-term rehab settings.

Deliver pulmonary and acute care, including admissions, discharges, assessments, and management of conditions like COPD, asthma, pneumonia, and respiratory failure.

Manage noninvasive ventilation, tracheostomy care, medication prescribing, and diagnostics interpretation.

Collaborate with interdisciplinary teams, participate in rounds, and educate patients and staff.

Ensure compliance with regulations and contribute to reducing hospital readmissions.

Qualifications: Active APRN license, NPI, BLS/ACLS, experience with respiratory care, EMR proficiency, and ability to work independently.

Details: Part-time, flexible schedule, $30K–$60K/year, benefits include PTO, paid onboarding, bonuses, and AI documentation support.
