Responsibilities include:
- Conducting adult and geriatric health assessments, reviewing medical history, diagnoses, and treatment plans
- Administering and ordering point-of-care and lab tests
- Collaborating with healthcare professionals, caregivers, and family members
- Providing health education and follow-up at members’ residences, including homes and facilities
Qualifications:
- Master’s Degree or equivalent experience with NP licensure
- Current state licensure, including prescriptive authority
- Board certification in relevant specialty (Adult, Gerontology, Family, or Emergency Care)
- Valid driver’s license and automobile insurance
- Strong communication skills and computer literacy
