We are seeking a Nurse Practitioner or Physician Assistant to join our interventional pain management team at multiple locations, including physical therapy, satellite clinics, and a surgery center.

Responsibilities:
- Assess, diagnose, and develop care plans for patients
- Order, interpret, and follow up on diagnostic tests
- Document patient care thoroughly
- Provide care according to standards and collaborate with team members
- Monitor and refer patients as needed
Qualifications:
- Current applicable state license
- National certification as NP or PA
- DEA license or ability to obtain one
- Basic Life Support certification
This full-time role offers a salary range of $125,000–$150,000 annually. Candidates must be willing to relocate to Sutherlin, OR before starting. The position involves in-person work in an outpatient setting.
