We are seeking a Part-Time Nurse Practitioner (NP) to support Skilled Nursing and Post-Acute facilities in Georgetown, SC.
Schedule & Environment:
– 3 days/week (Monday–Friday), 8:00 AM – 4:00 PM
– On-site at healthcare facilities
– No nights, weekends, or on-call duties
Responsibilities:
– Evaluate and manage admissions, follow-ups, and long-term care patients
– Provide chronic disease management and preventive care
– Collaborate with physicians and care teams
– Maintain accurate clinical documentation
– Serve as the onsite clinical representative
Qualifications:
– Active SC NP license and board certification
– Strong clinical judgment and documentation skills
– Experience in post-acute or outpatient settings preferred
– Interest in geriatrics and continuity of care valued
