We are seeking a Nurse Practitioner or Physician Assistant to join a multidisciplinary team providing comprehensive, patient-centered care to primarily geriatric patients with Medicare Advantage coverage.
Key responsibilities include:
- Managing a patient panel with an emphasis on quality care over quantity, typically seeing 14-16 patients daily
- Delivering physical, mental, and social health assessments and treatments
- Collaborating with healthcare providers to develop personalized care plans
- Supporting quality metrics and participating in performance incentives
Qualifications and skills required:
- Certification from AANP, ANCC, or NCCPA
- Current licensure through the Texas Board of Nurse Examiners or Medical Board
- Valid DEA license or ability to obtain one
- Experience managing Medicare-aged populations preferred
- Flexibility to work across multiple clinic locations in the greater San Antonio area
Preferred qualifications include:
- Understanding of managed care environments
- Bilingual in English and Spanish
- At least one year of clinical experience as a Nurse Practitioner or Physician Assistant
