Nurse Practitioner or Physician Assistant – New York State Traveler

Role Overview:
The Advanced Practice Clinician (NP or PA) provides comprehensive, patient-centered care to high-risk members within a home and community-based integrated care program. The role involves conducting assessments, supporting short-term rehabilitation, documenting clinical information, collaborating with care teams, and engaging in advanced care planning and patient education.

Key Responsibilities:

  • Perform assessments including medical history, vital signs, and screenings
  • Support members during rehabilitation episodes
  • Ensure accurate documentation and care coordination
  • Engage in advanced illness discussions and patient counseling

Qualifications & Skills:
  • Active certification (ANCC/AANP for NPs; NCCPA for PAs) and state licensure
  • Unrestricted licensure and DEA license (or ability to obtain)
  • Reliable transportation and active driver’s license
  • Preferred experience in geriatrics, long-term care, or home care setting
  • Proficiency with electronic health records and technology

Compensation & Benefits:
Salary ranges from $125,000 to $175,000 annually, plus incentives, comprehensive benefits, and development opportunities. The role requires 20% travel across New York state, with all expenses covered.
