Responsibilities include:
- Conducting comprehensive assessments with medical history review, vital signs, and screenings.
- Documenting diagnoses, collaborating with primary care providers, and addressing urgent situations.
- Educating members on health management, medications, and social determinants of health.
Qualifications:
- Active licensure as a Nurse Practitioner or Physician Assistant in the practice state.
- Unrestricted driver’s license and reliable transportation.
- Ability to navigate diverse home environments and perform assessments physically.
- For NPs: Certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
- For PAs: NCCPA certification and state licensure.
Preferred skills include:
- At least 1 year of clinical experience, particularly in geriatrics or home health.
- Proficiency with electronic medical records and technology.
- Strong communication skills with Medicare populations.
This role offers flexible scheduling, no on-call or weekend work, and travel expenses covered. Compensation ranges from $109,500 to $164,000, including base pay and incentives, with comprehensive benefits and growth opportunities.
