Provide advanced wound assessments, treatments, and follow-up care in various settings, including patients’ homes and care facilities. Collaborate with healthcare teams to ensure quality, continuous care, and educate patients and caregivers on wound management. Utilize EMR systems for accurate documentation and participate in care planning to reduce rehospitalizations. Work autonomously within a collaborative environment, with flexible full-time or part-time schedules, typically finishing by 5:00 pm, with no after-hours work.
