Nurse Practitioner – Hospice – Live Well Homecare

We are seeking a compassionate Nurse Practitioner specialized in Home Health and Hospice care to conduct patient assessments, manage complex conditions, and provide personalized care in patients’ homes and hospice settings.

Perform comprehensive patient evaluations and develop individualized care plans.

Monitor vital signs, manage symptoms, and provide catheter care.

Collaborate with interdisciplinary teams to ensure holistic, patient-centered care.

Utilize electronic health records like HomeCare Homebase for documentation.

Address behavioral health concerns and educate patients and families about care options.

Requirements include a valid Nurse Practitioner license with palliative or primary care focus, outpatient experience preferred, and proficiency in medical documentation and behavioral health management. Strong empathy and commitment to quality care are essential.
