Nurse Practitioner – Hospice

We are seeking a compassionate Nurse Practitioner with expertise in Home Health and Hospice care to provide comprehensive patient assessments, manage complex conditions, and deliver personalized care in patients’ homes and hospice settings.

Conduct individual assessments and develop tailored care plans.

Monitor vital signs, manage symptoms, and perform procedures like catheterization.

Collaborate with interdisciplinary teams and educate patients/families on care options.

Utilize EMR systems such as HomeCare Homebase for documentation.

Address behavioral health concerns and support holistic patient well-being.

Requirements include a valid Nurse Practitioner license, experience in outpatient or similar settings, proficiency with EMR, and a commitment to empathetic, patient-centered care.

**Position details:** Full-time or part-time, with compensation of $70-$79/hour, offering benefits like health insurance, 401(k), paid time off, and relocation assistance. The role involves travel to patients’ homes.
