Nurse Practitioner – Hospice

We are seeking a compassionate Nurse Practitioner specializing in Home Health and Hospice care to provide comprehensive patient assessments, manage complex medical conditions, and deliver personalized care in patients’ homes and hospice settings.

Conduct assessments, develop care plans, and monitor vital signs to improve patient comfort.

Perform catheter care and address behavioral health concerns.

Collaborate with interdisciplinary teams and educate patients and families about care options.

Utilize EMR/EHR systems like HomeCare Homebase for documentation.

Maintain adherence to HIPAA and infection control standards.

Requirements include a valid Nurse Practitioner license, experience in outpatient settings, and proficiency in medical documentation and patient-centered care.
