Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities.

Perform patient assessments, manage acute and chronic conditions, and develop treatment plans.

Order diagnostics, prescribe medications, and educate patients, families, and staff.

Collaborate with care teams to ensure seamless, patient-centered care.

Maintain accurate documentation and support quality improvement initiatives.

Qualifications: Current MN NP license, Board Certification, 1+ year related experience preferred, strong geriatric and primary care background, excellent communication skills.
