We are seeking a compassionate Nurse Practitioner to join our post-acute care team, providing high-quality care within the community. This role offers autonomy with strong clinical support in a collaborative, physician-led environment.
As an NP, you’ll manage your own patient panel, receive comprehensive onboarding, ongoing training, and oversight from a Medical Director. Your responsibilities include delivering patient-centered care, coordinating with healthcare team members, communicating effectively, and maintaining accurate documentation.
