Nurse Practitioner

Join a dedicated healthcare team as an Advanced Practice Provider, collaborating with Nurse Practitioners and Physician Assistants to deliver exceptional patient care in post-acute community facilities.

  • Manage and coordinate patient care, conduct rounds, and contribute to care decisions while maintaining accurate electronic records.
  • Communicate effectively with staff and physicians, write orders, and facilitate diagnostic and therapeutic procedures.
  • Provide compassionate, patient-centered care with strong organizational and time management skills.
  • Requires Nurse Practitioner license, DEA certification, and preferably one year of relevant experience in geriatrics or internal medicine.
