Nurse Practitioner – Live Well Homecare

Manage and coordinate patient care, conduct rounds, and contribute to care decisions while maintaining accurate electronic records.

Communicate effectively with staff and physicians, write orders, and facilitate diagnostic and therapeutic procedures.

Provide compassionate, patient-centered care with strong organizational and time management skills.

Requires Nurse Practitioner license, DEA certification, and preferably one year of relevant experience in geriatrics or internal medicine.
