We are seeking a dedicated part-time Nurse Practitioner to provide direct patient care in Skilled Nursing, Assisted Living, and Rehab facilities.
Responsibilities include evaluating and treating patients, following treatment plans, coordinating care with primary and specialty providers, updating families, managing documentation, and ensuring compliance with facility and PSG guidelines.
Qualifications include ANCC certification or equivalent, an active SC license preferred, RN certification, and experience in acute or hospital settings. Strong clinical skills, interpersonal abilities, and proficiency with EMR are essential.
