We are seeking a dedicated part-time Nurse Practitioner to provide direct patient care in Skilled Nursing, Assisted Living, and Rehab facilities.
Work closely with an attending MD to evaluate, treat, and manage patient care based on discharge summaries and treatment plans. Serve as the frontline provider, ensuring quality post-acute and long-term care while maintaining compliance and documentation within EMR. Coordinate with families, facility staff, primary care, and specialists to ensure seamless care delivery. Handle acute issues, review labs and radiology reports, and participate in quality improvement initiatives. Perform duties within a Monday-Friday schedule, 8 am-4 pm, three days a week. Qualifications include ANCC certification, SC license preferred, RN certification, and strong clinical and interpersonal skills. Benefits include career growth opportunities and a 401K plan.
