We are seeking a Nurse Case Manager to provide comprehensive healthcare services within a residential youth support setting.
Key responsibilities include:
- Medical case management, screening, education, and counseling
- Medication management and coordination with community healthcare providers
- Participating in a rotating on-call schedule, primarily via phone consultations
Qualifications:
- Bachelor or Associate Degree in Nursing from an accredited program
- Current Illinois RN licensure and valid driver’s license
- Experience with psychiatric care and children/adolescents preferred
- Strong organizational, communication, and clinical skills
