Role Overview:
As a Nurse Case Manager, you will support youth in emotional, psychological, and physical care within a residential setting. Your responsibilities include
- Medical case management
- Screening, education, and counseling
- Medication management
- Networking with community healthcare providers
- Participating in a rotating on-call schedule, primarily via remote phone consultations
Qualifications & Skills:
Candidates should hold a Bachelor or Associate Degree in Nursing, possess current Illinois RN licensure, and have strong organizational, communication, and clinical skills. Experience with psychiatric care or children/adolescents is preferred.
