Role Overview:
Seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health assessments for Medicare and other plan members, focusing on preventive care, education, and care gap closure. This non-prescribing, field-based role emphasizes collaboration with primary care providers, addressing medical, social, and environmental factors impacting health.
Responsibilities include:
- Conducting comprehensive assessments: medical history, medication review, vital signs, physical exams, screenings, and point-of-care tests
- Identifying diagnoses for care management and communicating findings to PCPs
- Recognizing urgent/emergent issues and intervening as needed
- Educating members on health conditions, medications, and compliance
- Addressing social determinants of health and providing referrals
Qualifications:
- Active state licensure and certification (ANCC/AANP for NPs, NCCPA for PAs)
- Unrestricted driver’s license and reliable transportation
- Ability to navigate home environments and perform assessments physically
- Compliance with licensing and practice regulations
Preferred Skills & Experience:
- 1+ years clinical experience, preferably in family, geriatric, or home health
- Proficiency with electronic medical records and technology
- Capability to transport equipment and navigate stairs
- Strong communication skills with geriatric or Medicare populations
