Role Overview:
As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform in-home health assessments for Medicare Advantage members, focusing on preventive care, education, and care gap closure. Your responsibilities include conducting comprehensive evaluations, documenting diagnoses, communicating with primary care providers, recognizing urgent situations, and addressing social determinants of health.
Key Duties:
- Perform medical history reviews, physical exams, and screenings during home visits
- Identify health issues, document diagnoses, and collaborate with primary care providers
- Educate members on health management and medication adherence
- Recognize and respond to urgent/emergent health situations
- Address social factors affecting health and provide appropriate referrals
Qualifications & Skills:
- Active certification (ANCC/AANP for NPs, NCCPA for PAs) and state licensure
- Unrestricted licensure and driver’s license with reliable transportation
- Ability to navigate home environments and perform assessments in varied settings
- Preferred: 1+ years clinical experience, proficiency with EMRs, and strong communication skills with diverse populations
