Role Overview: As a HouseCalls Advanced Practice Clinician, you will conduct in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care to improve health outcomes.
Responsibilities include:
Performing comprehensive assessments: medical history, medication reconciliation, vital signs, physical exams, screenings, and point-of-care testing.
Documenting diagnoses for care management and collaborating with primary care providers to address care gaps.
Recognizing urgent situations and providing appropriate interventions.
Educating members on health conditions, medications, and adherence.
Addressing social determinants of health and making referrals as needed.
Qualifications and Skills:
Active, unrestricted licensure as a Nurse Practitioner (ANCC/AANP) or Physician Assistant (NCCPA).
Valid driver’s license and reliable transportation for home visits.
Ability to navigate varied home environments and perform physical assessments.
Preferred: 1+ years of clinical experience, proficiency with electronic medical records, and strong communication skills with geriatric or Medicare populations.
