Role: Regional Medical Social Worker
Responsibilities: Conducts initial psycho-social assessments, develops individualized care plans, and establishes follow-up treatment goals. Identifies and reevaluates social, emotional, financial, and environmental factors impacting patient care. Communicates with internal contacts, utilizes community resources, educates patients and families on resource utilization, and provides discharge instructions.
Qualifications: Master’s degree in social work from an accredited institution or a baccalaureate degree with at least one year of healthcare social work experience. Preferably one year in home health care. Strong interpersonal and communication skills required.
Skills & Requirements: Knowledge of psycho-social principles, family dynamics, and community organization. Ability to work independently with travel as needed. Background checks and a drug-free workplace are mandatory.
