Role: Licensed Practical Nurse (LPN) or Med Aide (MA) providing patient-centered hospice care in a home setting.
Responsibilities:
- Implement and evaluate care for stable patients of all ages, focusing on comfort, symptom management, and dignity.
- Collaborate with interdisciplinary teams to support emotional comfort and safety.
- Administer treatments and medications, perform assessments, and interpret patient data.
- Ensure a safe, supportive environment by applying safety principles specific to home care.
Qualifications & Skills:
- Graduate from an accredited practical nursing program with an active LPN license.
- Knowledge of nursing principles, medication effects, safety precautions, and patient education.
- Preferred: One year of professional nursing experience; IV therapy skills (if required in the state).
- Basic Life Support (BLS) certification required.
- Possession of a valid driver’s license may be required.
Key Duties & Conditions:

- Participate in rotating shifts, including weekends and holidays.
