Role Overview

The LPN Medication Nurse provides licensed nursing support in senior care settings, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities include:

• Administering medications and treatments according to physician orders
• Following the Six Rights of Medication Administration to ensure safety
• Documenting in MAR/TAR accurately and in real time
• Monitoring symptoms and administering medications for relief
• Reporting refusals or adverse reactions promptly
• Conducting Service & Health Updates (SHUs)
• Transcribing physician orders and supporting care plan updates
• Observing and reporting physical, emotional, or cognitive changes
• Maintaining medication carts, completing narcotic counts, and ensuring compliance

Qualifications & Skills include:

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred

The role plays a critical part in preventing medication errors, early detection of health changes, and safeguarding resident safety through clinical precision and regulatory compliance.
