Role Overview

The Licensed Practical Nurse (LPN) Medication Nurse provides essential nursing support in a senior care setting, ensuring safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities include:

  • Administering medications and treatments according to physician orders
  • Following the Six Rights of Medication Administration
  • Accurately documenting in MAR/TAR in real time
  • Monitoring symptoms and administering medications for relief
  • Reporting refusals or adverse reactions promptly
  • Conducting Service & Health Updates (SHUs)
  • Transcribing physician orders and supporting care plan updates
  • Observing and reporting physical, emotional, or cognitive changes
  • Maintaining medication carts, completing narcotic counts, and ensuring compliance

Qualifications & Skills:

  • Active LPN license in good standing
  • CPR and First Aid certification
  • Strong assessment and documentation abilities
  • Experience in senior care or medication administration preferred

This role is vital for preventing medication errors, early detection of health changes, and safeguarding resident safety through clinical precision and regulatory adherence.
