LPN Medical Care Manager PT

Role Overview

The Licensed Practical Nurse (LPN) Medication Nurse provides licensed nursing support in a senior living environment, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities:

  • Administer medications and treatments according to physician orders, adhering to the Six Rights of Medication Administration.
  • Document accurately in MAR/TAR in real-time and transcribe physician orders to support care plan updates.
  • Monitor residents for symptoms, respond to adverse reactions or refusals, and report changes promptly.
  • Conduct Service & Health Updates (SHUs), maintain medication carts, and ensure compliance with regulatory standards.
  • Observe physical, emotional, or cognitive changes in residents and report findings appropriately.

Qualifications & Skills:

  • Active LPN license in good standing.
  • CPR and First Aid certifications.
  • Strong assessment, observation, and documentation skills.
  • Preferred experience in senior care or medication administration.

The role emphasizes clinical precision, resident safety, and regulatory adherence, contributing to the overall quality of life for seniors in care.
