LPN Medical Care Manager FT

Role Overview

The LPN Medication Nurse provides licensed nursing support in senior care by safely administering medications, monitoring residents’ health, and ensuring accurate clinical documentation.

Key Responsibilities:

  • Administer medications and treatments according to physician orders, following the Six Rights of Medication Administration
  • Document accurately in MAR/TAR in real-time
  • Monitor symptoms, report adverse reactions, and support symptom relief through medication administration
  • Conduct Service & Health Updates (SHUs), transcribe physician orders, and support care plan updates
  • Observe and report physical, emotional, or cognitive changes in residents
  • Maintain medication carts, perform narcotic counts, and ensure compliance with standards

Qualifications & Skills:

  • Active LPN license in good standing
  • CPR and First Aid certification
  • Strong assessment and documentation skills
  • Experience in senior care or medication administration preferred

Key Duties: Ensuring resident safety through clinical precision, preventing medication errors, and early health change detection while adhering to regulatory standards.
