LPN Medical Care Manager

Role Overview

The LPN Medication Nurse provides licensed nursing support focused on safe medication administration, health monitoring, and accurate clinical documentation in a senior living environment.

Key responsibilities include:

  • Administering medications and treatments according to physician orders, ensuring compliance with the Six Rights of Medication Administration.
  • Documenting medication records accurately and in real-time within MAR/TAR systems.
  • Monitoring residents for symptoms, administering medications for relief, and reporting any refusals or adverse reactions promptly.
  • Conducting Service & Health Updates (SHUs), transcribing physician orders, and supporting updates to care plans.
  • Observing and reporting physical, emotional, or cognitive changes in residents.
  • Maintaining medication carts, performing narcotic counts, and ensuring regulatory compliance.

The role is vital in preventing medication errors, early detection of health changes, and safeguarding resident safety through clinical precision.

Qualifications include:

  • Active LPN license in good standing
  • CPR and First Aid certification
  • Strong assessment and documentation skills
  • Experience in senior care or medication administration is preferred
