Role Overview: The LPN Medication Nurse provides licensed nursing support in senior care by administering medications, monitoring health, and ensuring accurate documentation.
Key Responsibilities:
- Administer medications and treatments following physician orders
- Follow the Six Rights of Medication Administration to ensure safety
- Accurately document in MAR/TAR in real-time
- Monitor symptoms and administer medications for relief as ordered
- Report any refusals or adverse reactions promptly
- Conduct Service & Health Updates (SHUs)
- Transcribe physician orders and support care plan updates
- Observe and report physical, emotional, or cognitive changes
- Maintain medication carts, perform narcotic counts, and ensure compliance standards
Qualifications & Skills:
- Active LPN license in good standing
- CPR and First Aid certification
- Strong assessment and documentation skills
- Experience in senior care or medication administration preferred
