Role Overview: As an LPN Medication Nurse, you provide licensed nursing support by administering medications, monitoring residents’ health, and maintaining accurate clinical documentation.
Key Responsibilities:
- Administer medications and treatments following physician orders and the Six Rights of Medication Administration
- Document accurately in MAR/TAR in real time
- Monitor symptoms, administer medications for relief, and report any adverse reactions or refusals
- Conduct Service & Health Updates (SHUs) and support care plan updates by transcribing physician orders
- Observe and report physical, emotional, or cognitive changes in residents
- Maintain medication carts, perform narcotic counts, and ensure compliance with safety standards
Qualifications & Skills:
- Active LPN license in good standing
- CPR and First Aid certifications
- Strong assessment, observational, and documentation skills
- Experience in senior care or medication administration is preferred
