LPN Med Care Manager

Role Overview

The Licensed Practical Nurse (LPN) Medication Nurse provides licensed nursing support in a senior living environment, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities:

  • Administer medications and treatments according to physician orders, adhering to the Six Rights of Medication Administration
  • Document all procedures accurately in MAR/TAR in real time
  • Monitor residents’ symptoms, administer medications for relief, and report any refusals or adverse reactions
  • Conduct Service & Health Updates (SHUs), transcribe physician orders, and support care plan updates
  • Observe and report physical, emotional, or cognitive changes in residents
  • Maintain medication carts, complete narcotic counts, and ensure compliance with regulations

Qualifications & Skills:

  • Active LPN license in good standing
  • CPR and First Aid certification
  • Strong assessment and documentation skills
  • Preferred experience in senior care or medication administration

The role is vital in preventing medication errors, early health change detection, and safeguarding resident safety through clinical accuracy and adherence to regulations.
