Role Overview
The LPN Medication Nurse provides licensed nursing support to senior residents, focusing on safe medication administration, health monitoring, and accurate clinical documentation.
Key Responsibilities:
- Administer medications and treatments according to physician orders
- Follow the Six Rights of Medication Administration
- Document accurately in MAR/TAR in real time
- Monitor symptoms and administer medications for symptom relief
- Report refusals or adverse reactions promptly
- Conduct Service & Health Updates (SHUs)
- Transcribe physician orders and support care plan updates
- Observe and report physical, emotional, or cognitive changes
- Maintain medication carts, complete narcotic counts, and ensure compliance
Qualifications & Skills:
- Active LPN license in good standing
- CPR and First Aid certification
- Strong assessment and documentation skills
- Experience in senior care or medication administration preferred
