LPN Med Care Manager

Role Overview

The LPN Medication Nurse provides licensed nursing support to ensure safe and accurate medication administration, health monitoring, and proper clinical documentation in a senior living environment.

Key Responsibilities:

  • Administer medications and treatments following physician orders and the Six Rights of Medication Administration.
  • Accurately document in MAR/TAR in real-time and support care plan updates.
  • Monitor resident symptoms, report adverse reactions or refusals promptly, and observe physical, emotional, or cognitive changes.
  • Maintain medication carts, conduct narcotic counts, and ensure compliance with regulatory standards.
  • Conduct Service & Health Updates (SHUs) and transcribe physician orders.

Qualifications & Skills:

  • Active LPN license in good standing with CPR and First Aid certification.
  • Strong assessment, observation, and documentation skills.
  • Experience in senior care or medication administration is preferred.

This role plays a vital part in safeguarding resident safety, preventing medication errors, and supporting health outcomes through clinical accuracy and regulatory adherence.
