Role Overview

The LPN Medication Nurse provides essential licensed nursing support in a senior care environment, focusing on safe medication administration, health monitoring, and precise clinical documentation.

Key Responsibilities:

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Accurately document in MAR/TAR in real-time
• Monitor residents for symptoms, adverse reactions, and health changes
• Transcribe physician orders and support care plan updates
• Report physical, emotional, or cognitive changes promptly
• Maintain medication carts, conduct narcotic counts, and ensure compliance with regulations
• Conduct Service & Health Updates (SHUs) and document appropriately

Qualifications & Skills:

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment, observation, and documentation skills
• Experience in senior care or medication administration preferred

The role is vital in preventing medication errors, early detection of health issues, and ensuring resident safety through clinical accuracy and adherence to regulatory standards.
