The Clinical Care Coordinator II provides patient care tailored to age-specific needs and assists in managing patient visits under the supervision of a provider.
This role involves daily collaboration with physicians at the outpatient internal medicine office located in Waldemere Medical Plaza on the hospital campus.
Required qualifications include BLS/CPR certification within 30 days of hire and graduation from an accredited nursing or diploma program.
Preferred qualifications encompass at least one year of outpatient clinical experience, familiarity with electronic health records and practice management software, and proficiency in Microsoft Office programs.
Licensure as a Florida Licensed Practical Nurse is required. The schedule is Monday through Friday, with no weekends or holidays. Background screening through the Florida Care Provider Background Screening Clearinghouse is mandatory for all care providers.
