Role: Licensed Vocational Nurse (LVN) in community health clinics.
Responsibilities:
- Support healthcare providers with rooming, taking vital signs, administering injections and medications, and performing basic procedures.
- Maintain accurate electronic health record (EHR) documentation.
- Provide patient education and engage in care coordination to enhance health outcomes.
- Conduct phone triage to improve patient access and manage care needs.
- Collaborate within an integrated team including medical, behavioral health, dental, and care management staff.
- Participate in quality improvement initiatives and promote equitable care for diverse populations.
Qualifications & Skills:
- Valid LVN license.
- Strong patient-centered care skills with compassionate approach.
- Ability to perform clinical procedures accurately and efficiently.
- Excellent communication and documentation skills.
