Job Summary
Seeking a Licensed Practical Nurse (LPN) to serve as a patient navigator, guiding new patients through program entry, providing education, and ensuring continuity of care. The role involves clinical support, collaboration with healthcare providers, and communication with patients via multiple channels. Responsibilities include assessing patient needs, coordinating activities, maintaining records, and supporting provider functions. The position requires a minimum of 2 years LPN experience, with Cardiovascular and Adult Echocardiography expertise preferred.
Key Duties
- Utilizes clinical knowledge to facilitate patient program entry.
- Performs patient evaluations and documents needs primarily through phone or documentation review.
- Collaborates with healthcare teams to coordinate care plans.
- Communicates effectively with patients and providers.
- Coordinates patient activities, education, and support services.
- Maintains systems for follow-up and surveillance.
- Serves as a liaison for scheduling and patient navigation procedures.
- Investigates patient concerns and manages clinical supplies.
- Conducts medical record reviews and provides nursing support during procedures.
- Inspects equipment and maintains exam and treatment rooms.
Qualifications & Skills
- Graduate from Specialty Training Program-Nursing.
- Strong communication, organization, and teamwork skills.
- Experience in cardiovascular and adult echocardiography preferred.
