Role Summary: A Licensed Practical Nurse serving as a patient navigator, responsible for guiding new patients, providing education, and ensuring continuity of care. The role involves collaborating with healthcare providers, coordinating patient activities, and supporting diagnosis and treatment processes.
Key Responsibilities:
- Facilitate patient entry into the program through clinical knowledge and documentation review.
- Coordinate patient care plans, including evaluation, treatment, and surveillance in collaboration with providers.
- Communicate effectively with patients via mail, phone, and in person to convey pertinent information.
- Serve as a point of contact for providers, schedule patients, and support program activities like support groups and educational series.
- Provide limited education to patients and families based on individual needs.
- Maintain patient records, review medical data, and assist with procedures and nursing care.
- Ensure proper operation and maintenance of clinical equipment and exam rooms.
- Investigate patient concerns related to appointments, communication, and billing.
Qualifications & Skills: Minimum of 2 years LPN experience, with preferred cardiovascular and adult echocardiography expertise. Strong communication, coordination, and clinical skills are essential.
Position Details: Full-time in a clinical environment, with responsibilities encompassing patient care, program coordination, and system management in accordance with policies and procedures.
Education: Graduate from a Specialty Training Program in Nursing (Required).
